Repeat Prescription Order Form

To request a repeat supply of long-term medication for your pet, please fill in the form below.

We will reply by e-mail when the medication is ready to collect, or if there are any issues we need to resolve. Please note that we are only able to prescribe medication to animals under our care, and regular re-examinations are required by our regulators. We will normally prescribe one month’s medication at a time, though this can be extended by arrangement.

Medications should be paid for on collection at the clinic.
